Changes to Advance Care Planning & Medical Order for Scope of Treatment Policy & Forms – LTC & AL: Advance Care Planning (ACP) documents are centrally located in an electronic single source of truth in Fraser Health. In-person Skin & Wound Care classes are restarting in Fall 2023 for LTC & AL: Topics in include: the interdisciplinary team approach in skin/wound care, common risk factors for skin breakdown, factors that influence wound healing, the staging of Pressure injuries, and more. Save our Skin (SOS) Education for LTC: Collaborative is a partnership where Long Term Care (LTC) communities come together with Fraser Health (FH) to increase their knowledge, skills and abilities in pressure injury prevention and implementation of strategies to reduce pressure related injuries and ultimately improve resident quality of life.
